Pediatric dentist said that exposure of the pulp is caused most commonly by caries,but may also occur during cavity preparation or by fracture of the crown.Pulp exposures caused by caries occur more frequently in primary than in permanent teeth because the former have relatively large pulp chambers,more prominent pulp horns,and thinner enamel and dentine.
Exposure of the pulp by caries is invariably accompanied by infection of the pulp,and a traumatic exposure may quickly become infected by carious debris or saliva.The infected pulp becomes inflamed,and necrosis may result;if infection spreads to the alveolar bone the developing permanent tooth may be affected.Furthermore,although the inflammation may remain sub-acute or chronic and therfore give the patient little or no pain,the process may become acute at any time.For these reasons,a primary tooth with a pulp exposure should not be left untreated;a choice must be made between conservation by some forms of pulp treatment.The methods of treatment include pulp capping,pulpotomy(partial pulpectomy)and radical treatment.Pulpotomy and radical treatment are used widely in the clinic for pulp involvement patients.
