Pulpotomy is a procedure in which the entire coronal pulp is removed,with the aim of removing all infected pulp tissue;the radicular pulp is then treated in different ways,according to the technique employed.Pulpotomy is performed mainly in vital teeth with pulp exposures larger than those considered suitable for pulp capping.
In permanent teeth,the calssical pulpotomy technique involves placing calcium hydroxide in the base of the pulp chamber after having removed the coronal pulp.This technique has been used with success in the treatment of vital pulps in premolars and molars.However,in primary teeth this method has been found to be relatively unsuccessful,often being accompanied by internal resorption of the roots.Therefore,other pulpotomy methods have been developed for primary molars:vital pulpotomy using formocresol,and devitalisation pulpotomy.These are alternative methods for teeth with vital pulps.
For non-vital pulps,pulpectomy and root canal therapy is the ideal treatment,but since this is often not practicable for primary molars,a non-vital pulpotomy method is advocated.
